Dosing Guide

Xarelto 15 mg and 20 mg should be taken with food
Xarelto 15 mg and 20 mg should be taken with food.
Xarelto 2.5 mg and 10 mg can be taken with or without food.

a Use with caution when patient is concomitantly receiving other medicinal products that increase rivaroxaban plasma concentration.
b Not recommended in patients with CrCl <15 ml/min; use with caution in patients with CrCl 15–29 ml/min and in patients with CrCl 30–49 ml/min when concomitantly receiving other medicinal products that increase rivaroxaban plasma concentration.
c In patients with CrCl 15–49 ml/min 15 mg OD should be considered if the patient’s assessed risk of bleeding outweighs the risk of recurrent PE/DVT.
d In combination with standard antiplatelet therapy of ASA alone or ASA in combination with clopidogrel or ticlopidine, in adults with elevated cardiac biomarkers: Troponin-I/T; creatine kinase-muscle and brain isoenzyme (CK-MB).
